DIA Liability Form
(Department of Intercollegiate Athletics) | This form serves as a Release, Consent to treat, and Indemnification agreement between the student athlete and BGSU athletic department. |

BGSU Health History Form | This form aids in obtaining a complete medical history of each student athlete. |

BGSU Physical Examination Form | Physical exams should be given to a reliable family doctor to fill out. An additional "BGSU sports medicine physical exam" will assessed upon arriving to campus. |

Authorization to Release Information Form | This is a necessary release to be able to communicate wit coaches, ATC staff, doctors, family, Professional scouts and BGSU media. |
| BGSU Nutritional Supplement Disclosure Form | This form is used to identify any supplementations that are being used by our athletes. Please note that no supplementation of any sort may be taken without the approval of the BGSU Sports Medicine Staff. |
